Solution:

1) Click the 'Start' button to display the 'Start' menu.
2) Select 'Programs' and select 'Accessories'. (The 'Accessories' submenu appears.)
3) Select 'Fax'. (The 'Fax' submenu appears.)
4) Select 'Cover Page Editor' from the 'Fax' submenu. (The 'Cover Page - Fax Cover Page Editor' window appears.)
5) Add the recipient information to the cover page:

a) Select the 'Insert' menu and select 'Recipient'. (The 'Recipient' submenu appears.)
b) Select the desired type of recipient information from the submenu.

c) Repeat steps 5)a) and 5)b) to include more information about the recipient on the fax cover.
NOTE: 'Recipient Name' is the minimum required information for the recipient.

6) Add the sender information to the cover page:

a) Select the 'Insert' menu and select 'Sender'. (The 'Sender' submenu appears.)
b) Select the desired type of sender information from the submenu.
c) Repeat steps 6)a) and 6)b) to include more information about the sender on the fax cover.
NOTE 1: 'Sender Name' and 'Sender Fax number' are the minimum required information for the sender.
NOTE 2: The 'Cover Page Editor' automatically fills in the sender fields with information from the 'User Properties' sheet in 'Fax Setup'.

7) (Optional) Add some information about the message to be sent:

a) Select the 'Insert' menu and select 'Message'. (The 'Message' submenu appears.)
b) Select the desired type of information regarding the message from the submenu.
c) Repeat steps 7)a) and 7)b) to include more information about the message on the fax cover.

8) Save the fax cover in the desired folder:

a) Select the 'File' menu and select 'Save'. (The 'Save As' dialog box appears.)
b) Select the drive to which to save the file from the 'Save in' drop-down list.
c) Select the folder in which to store the file from the middle of the 'Save As' dialog box.
d) Accept or change the name of the file in the 'File name' box.
e) Click 'Save' to save the fax cover sheet.
